Appliance Repair Invoice Template: Diagnostic Fee, Parts by Model Number and Warranty Lines

Updated November 4, 2026

By Moni Mindich, founder, 20 years in home improvement

An appliance repair invoice does three jobs: it gets you paid, it backs your parts and labor warranty, and it is the service history for the next call to the same machine.

Quick answer: An appliance repair invoice template should record the appliance (brand, model and serial number), the complaint and what you found, the diagnostic fee and whether it is credited to the repair, labor, each part with its part number and price, any second trip for a part on order, tax where it applies, your parts and labor warranty, and payment. For home-warranty jobs, add the claim number and the service fee collected.

What an appliance repair invoice has to prove

An appliance invoice is read three times. The customer reads it on the day to see why the dryer cost what it did. You read it again in 60 days when the same customer calls saying it stopped heating, and the invoice is the only proof of which part you replaced and when your warranty ends. And the manufacturer or home-warranty company reads it if a part fails under its warranty. Every line below is there for one of those readers.

The template, line by line

  1. Your company, license number where your state requires one, invoice number and date.
  2. Customer name, service address and phone.
  3. Appliance. Type, brand, model number and serial number. Take a photo of the tag and attach it.
  4. Complaint and finding. In the customer's words, then yours: "Dryer runs, no heat. Thermal fuse open, heating element open."
  5. Diagnostic or trip fee, and a line showing it credited if the customer approved the repair.
  6. Labor. Flat rate per repair or hours at your rate, whichever you quoted.
  7. Parts. One line per part with the part number, quantity and price. Mark OEM or aftermarket.
  8. Second trip, if a part had to be ordered.
  9. Sales tax, only on what is taxable in your state; many states tax parts and not labor, some tax both.
  10. Warranty. Parts and labor period in days, and what it excludes.
  11. Payment and signature. Paid by, date, and the customer's approval of the repair.

Worked example: dryer not heating

These are our assumptions for illustration, not market prices. Electric dryer, no heat. The technician finds an open thermal fuse and an open heating element, has both on the truck, and finishes on the first visit.

LineDetailAmount
Diagnostic visitNo heat, tested fuse, element, thermostat$95.00
Diagnostic credited to repairCustomer approved repair on site− $95.00
LaborFlat rate, replace element and fuse$165.00
Part: heating elementOEM, part number as listed for this model$148.00
Part: thermal fuseOEM$18.00
Dryer vent checkRestricted vent found, cleared at the machine; full vent cleaning recommended$45.00
Sales tax8% on parts only (assumption: labor not taxed in this state)$13.28
Total$165 + $148 + $18 + $45 + $13.28$389.28

Two notes on this example. First, the vent line matters: a blown thermal fuse often means restricted airflow, and if you replace the fuse without saying so, the dryer comes back. Writing it on the invoice protects your warranty. Second, if the element had to be ordered, the invoice would carry a second-trip line and the customer would get a deposit invoice on the first visit. With a $55 second-trip charge and no vent work, the same repair totals $165 + $148 + $18 + $55 + $13.28 = $399.28.

Flat rate or hourly on the labor line?

Customers compare appliance invoices by the total, so most repair companies quote labor as a flat rate per repair type: replace a dryer element, a washer drain pump, a dishwasher control board. Flat rates are faster to quote on the phone and avoid arguments about a slow job. Build each one from your average hours for that repair, your hourly cost and your margin, and review them twice a year against the actual times your technicians log. Keep hourly billing for the jobs you cannot predict, such as intermittent faults, and say so on the estimate before the work starts.

Home-warranty and manufacturer-warranty jobs

For home-warranty dispatches, the invoice needs the claim or work order number, the service fee you collected from the homeowner, the authorization number for the repair, and the part numbers exactly as you submitted them. Bill the warranty company only what it authorized; anything the homeowner chose to add (a vent cleaning, a second appliance) goes on a separate customer invoice so the warranty company's payment and the homeowner's payment never get mixed. For manufacturer warranty work, follow the manufacturer's claim form and keep the serial-number photo.

Sealed-system repairs on refrigerators and freezers (compressors, leaks, recharges) need an EPA Section 608 certified technician to handle the refrigerant. List the refrigerant type and amount on the invoice.

Estimate first, invoice second

The invoice should match an estimate the customer approved before the work: the same labor, the same parts, the same diagnostic credit. Our appliance repair estimate template uses the same lines, and the appliance repair estimate calculator prices the labor and parts markup from your own costs.

Common questions

What should an appliance repair invoice include?

Your company and license details, the customer, the appliance brand, model and serial number, the complaint and finding, the diagnostic fee and any credit, labor, each part with its part number, any second trip, tax where it applies, your warranty period and the payment.

Should the diagnostic fee be credited to the repair?

Many companies credit it when the customer approves the repair, which helps close the job. Either way, show it on the invoice: the fee on one line and the credit on the next.

How do I invoice a home-warranty appliance repair?

Include the claim or work order number, the authorization number, the service fee collected from the homeowner and the authorized parts exactly as submitted. Put anything the homeowner added on a separate invoice.

Do I need a certification to bill for refrigerant work?

Yes. Handling refrigerant in sealed-system repairs requires an EPA Section 608 certified technician. List the refrigerant type and amount on the invoice.

Turn the hours this takes into minutes of talking.

Home service businesses answer only about half their calls (Invoca, July 2026), and owners spend about 11 to 16 hours/week on admin. Knowing what to do is the easy part; the money leaks out in the calls you can't take, the quotes nobody chases and the invoices that sit unpaid. Unmanually is a 24/7 AI Office Manager for appliance repair that you run by voice from your phone:

  • Answers your business line 24/7, quotes from your price list and books the job on the live call, so the customer doesn't ring the next company on Google.
  • Sends the quote, follows up until they say yes, then sends the invoice and chases it until it's paid.
  • Texts confirmations and reminders, keeps your calendar and remembers every price and promise.
  • Plus a free website for your business, set up for you. On a plan, every form fill gets a call back from your AI Office Manager in about a minute.
