Good Faith Estimate Chiropractic Template: The CMS Timing Rules and a Worked 12-Visit Care Plan

Updated October 28, 2026

By the Unmanually team, written from the sources cited in the text

If you need a good faith estimate chiropractic template, you are probably seeing cash or self-pay patients, and the No Surprises Act says they are owed an itemized estimate on a deadline. Below are the CMS timing rules, a fill-in template and a worked estimate for a 12-visit care plan.

Quick answer: A chiropractic good faith estimate goes to any patient not using insurance to pay. It must be itemized with expected charges and given within 1 business day if the visit is booked 3 to 9 business days out, or within 3 business days if booked 10 or more days out or on request. Estimate the full care plan, because a bill $400 or more above the estimate can be disputed.

When a chiropractic office needs a good faith estimate

Under the No Surprises Act, a patient who is not using health insurance to pay for care, whether uninsured or choosing to self-pay, has the right to a good faith estimate of expected charges (CMS, Good Faith Estimate guide (updated October 5, 2026)). For a chiropractic practice, that is most cash patients and anyone who pays out of pocket for a care plan. The CMS rules on timing are specific:

When the visit is scheduledWhen the estimate is due
0 to 2 business days aheadNo estimate required
3 to 9 business days aheadWithin 1 business day of scheduling
10 or more business days aheadWithin 3 business days of scheduling
Patient asks before schedulingWithin 3 business days of the request

The estimate must be itemized with the expected charges, and given on paper or by email, as the patient prefers. If the final bill comes in at least $400 more than the estimate, the patient can start a dispute within 120 calendar days of the initial bill (CMS, dispute a bill). This guide is written from those CMS pages; it is not legal advice, so check the federal rules and your state board for anything specific to your practice.

Worked example: a 12-visit cash care plan

The fees below are assumptions for illustration; use your own fee schedule. Our free chiropractic fee calculator helps set cash fees that cover your costs.

ServiceQuantityExpected chargeTotal
New patient exam1$85.00$85.00
X-rays, in office (2 views)1$70.00$70.00
Spinal adjustment, 1 to 2 regions12$55.00$660.00
Therapeutic exercise, 15 minutes6$35.00$210.00
Re-exam at week 61$50.00$50.00
Total expected charges$1,075.00

Two practical points come out of this example. First, estimate the full plan you are recommending, not just the first visit: if the patient later receives the 12 adjustments and the bill is $400 or more above an estimate that only covered the exam, that is a dispute you invited. Second, if you sell a prepaid package at a discount, show the package price and what it includes so the estimate and the receipt match.

Common questions

Do chiropractors have to give good faith estimates?

Yes, for patients who are not using health insurance to pay for care, including uninsured and self-pay patients, under the No Surprises Act rules described by CMS.

When is a good faith estimate due?

Within 1 business day if the visit is scheduled 3 to 9 business days ahead, within 3 business days if it is scheduled 10 or more business days ahead, and within 3 business days of a request made before scheduling. No estimate is required for visits booked 0 to 2 business days ahead.

Should the estimate cover the whole care plan?

Estimating the full recommended plan is safer, because a patient billed at least $400 more than the estimate can start a dispute within 120 days of the initial bill.

Can I email the good faith estimate?

Yes. CMS says it must be given on paper or by email, as the patient prefers.
