Chiropractic Fee Calculator

By Moni Mindich, founder, written from the cited sources

Updated October 22, 2026

This chiropractic fee calculator works out what a visit has to bring in, from the practice's yearly overhead, the doctor's pay, patient hours and how many visits fit in an hour. Then it blends in your payer mix: what insurance really pays per visit after collection and billing cost, and the cash visit price that brings the average to target. You get the cost per visit, the target average collected per visit, the cash price needed and the yearly revenue at that target.

Quick answer: Visits per year = patient hours per week × weeks × visits per hour. Cost per visit = (overhead + doctor pay) ÷ visits per year. Target average = cost ÷ (1 − margin). Cash price needed = (target − insurance share × insurance net per visit) ÷ cash share, where insurance net = reimbursement × collection − billing cost.

You just spent 10 minutes on one visit. Unmanually turns that into a 10-second voice note.

Owners spend about 11 to 16 hours/week on admin, and a lead reached within an hour is nearly 7 times more likely to turn into a real conversation (HBR). The math is the easy part; the money leaks out in the calls you can't take, the follow-ups nobody sends and the invoices that sit unpaid. Unmanually is a 24/7 AI Office Manager for chiropractors that you run by voice from your phone:

  • Answers your business line 24/7, quotes from your price list and books the visit on the live call, so the customer doesn't ring the next company on Google.
  • Sends the quote, follows up until they say yes, then sends the invoice and chases it until it's paid.
  • Texts confirmations and reminders, keeps your calendar and remembers every price and promise.
  • Plus a free website for your business, set up for you. On a plan, every form fill gets a call back from your AI Office Manager in about a minute.

How to use this chiropractic fee calculator

  1. Add up a year of overhead. Everything except the doctor's own pay: staff, rent, equipment, software, malpractice, marketing and billing.
  2. Enter the doctor's pay and the real schedule. Patient hours, weeks open and visits per hour at your actual fill rate, not a full book.
  3. Enter your payer mix from your own data. Share of cash visits, the average allowed amount per insurance visit from your EOBs, the share actually collected and the cost of billing.
  4. Set the profit after doctor pay. The calculator gives the target average and the cash price that reaches it.
  5. Change the mix. See how much a shift toward cash visits, or better collection, moves the price you need.

The formula

visits per year = patient hours per week × weeks × visits per hour
cost per visit = (overhead + doctor pay) ÷ visits per year
target average per visit = cost per visit ÷ (1 − profit %)
insurance net per visit = allowed amount × collection − billing cost
cash price needed = (target − insurance share × insurance net) ÷ cash share

Worked example

These are assumptions for illustration, not market figures. Use your own books and EOBs.

A practice with $250,000 of overhead, $120,000 of doctor pay, 30 patient hours/week for 48 weeks at 4 visits an hour, 40% cash visits, insurance allowing $48 a visit on average with 90% collected and $4 of billing cost, and a 10% profit target after doctor pay.

Visits per year: 30 × 48 × 4 = 5,760. Cost per visit: $370,000 ÷ 5,760 = $64.24, or $256.94 per patient hour. Target average: $64.24 ÷ 0.90 = $71.37. Insurance nets $48 × 90% − $4 = $39.20 a visit, $32.17 short of target. To make the average work, cash visits need ($71.37 − 60% × $39.20) ÷ 40% = $119.63. Revenue at target is $411,111/year.

Fill the schedule to 5 visits an hour and the cost per visit drops to $51.39; the cash price needed falls to $83.95.

How to set chiropractic fees

Start with cost per visit

Every visit has to carry its share of rent, staff, equipment, marketing and the doctor's pay. Divide a year of those costs by the visits you really deliver, at your real schedule fill, and you have the floor. Fees set below it lose money on every adjustment no matter how full the day looks.

Insurance pays what it pays

Your fee schedule is not what you collect on insurance visits. The allowed amount, denials, write-offs and the cost of billing decide the net. Pull the average from your EOBs by payer, then compare it with your cost per visit. Payers far below cost deserve a hard look at the contract.

Cash plans and care plans

Many practices offer cash visits and prepaid care plans for patients without coverage or past their visit limits. The calculator shows the cash price your mix needs to hit the target. Check how your state's rules and payer contracts treat cash prices and discounts before you set them; this is not legal advice, and a healthcare attorney or consultant can help.

Schedule fill is the hidden lever

Visits per hour is really visits booked and kept per hour. No-shows, gaps in the schedule and patients who drop off their care plan raise the cost of every visit that does happen. Reminders, easy rebooking and fast answers to new patient calls do as much for the numbers as a fee change.

Review fees every year

Overhead rises, payer allowed amounts change and the mix shifts. Re-run the numbers yearly, and track collected revenue per visit, not just billed charges, so the practice sees the real average. Watch the new patient exam separately: it takes longer, often carries X-rays, and sets up the care plan that pays for the practice.

Front desk time counts

Insurance verification, claims, denials and patient balance calls take staff hours. If the billing cost per insurance visit is low on paper but the front desk spends hours on the phone with payers, that time belongs in the number.

Quote calls come in while you're on a job

New patients in pain call the first clinic that can see them today. Unmanually is an AI Office Manager that works 24/7 and that you run by voice: the AI Office Manager answers, asks the complaint, insurance or cash and preferred times, books the first visit and texts the confirmation and intake link, sends reminders to cut no-shows, then sends statements and chases balances. FAQ

Common questions

How do chiropractors set their fees?

Start from cost per visit (overhead plus doctor pay ÷ visits per year), add a profit target, then check what insurance really pays and what cash visits must cost to reach the average. The calculator does each step.

How much should a chiropractor charge for a cash visit?

Enough that your blended average reaches your target. With the worked example's assumptions (40% cash visits, insurance netting $39.20 a visit, 10% profit) the cash price needed is about $119.63. Your own mix and costs give your number.

How many patient visits does a chiropractor need per year?

Divide overhead plus pay by what each visit nets. The calculator shows visits per year from your hours and visits per hour, and the cost each one must cover.

What is a chiropractor's cost per visit?

Overhead plus doctor pay divided by visits per year. In the example, $370,000 ÷ 5,760 visits = $64.24 a visit.

Can a chiropractor charge cash patients less than insurance patients?

Rules differ by state and payer contract. Check with a healthcare attorney or consultant before setting cash prices or discounts; the calculator only does the math.
